Responsibilities

The successful Case Manager will work collaboratively and effectively as a member of the primary service navigator case management team, conducting screening, diagnostic, functional and psychosocial assessments;  using results of assessments to develop, monitor, and evaluate professional service plans with measurable goals that are responsive to the needs revealed by the assessments, and to client preferences; providing counseling services and guidance at the agency and in the community to help clients achieve the goals of the service plan;  and providing prompt and effective services that respond to individual’s needs on their path to independence.

1. Manage a caseload of clients with visual impairments by conducting screening, vision function, diagnostic, functional and  psychosocial assessments, and completes a comprehensive, accurate,  and professional report of assessment results. Document information in agency database and health record. 

2. Duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the services needed for the client to gain independence.

3. Develop and implement individual service plan through agency, home or field visits.

4. Work cooperatively with agency staff in developing and implementing client's service plan.

5. Make referrals to Opportunities for Ohioans with Disabilities (OOD) with appropriate follow-up. 

6. Initiate and coordinate intra and inter Agency referrals, track and manage services. 

7. Follow-up on agreed upon service referrals, ensuring that clients' needs are being met in order to assist the client in reaching independence.

8. Maintain daily case notes and weekly statistical reports for department and enter information into database in a timely manner. 

9. Acts in accordance with federal and state rules, regulations, laws, and administrative rules pertaining to the provision of Social Work, Counseling and Case Management services, including those concerning the privacy and confidentiality of protected health information; reporting suspected abuse, neglect, and exploitation; and protecting the safety and health of clients and the community. Update all records to meet CARF standards.

Required Skills and Experience

A.     Education: Minimum of a Bachelor's Degree in Social Work, Counseling, Vocational Rehab, Blind Rehabilitation. Masters in Social Work, Counseling or Vocational Rehab, preferred. Licensure to practice Social Work or Counseling in the State of Ohio a plus (LSW, LISW, PC or PCC).

B.     Experience:     Must have at least two years’ experience in Social Work, Counseling, Case Management or Blind/Vision Impairment Rehabilitation. 

C.     Skills:               
Excellent written and oral communication skills.  Good assessment skills.  Ability to work well under pressure.  Able to work well with visually impaired individuals, including the elderly.  Should have a working knowledge of community resources.  Ability to provide direct involvement and guidance to individuals, their families, or groups. Develop individual service plans for client’s independence and monitor the progress of individual clients.
Knowledge of CSC policies, procedures, with a demonstrated willingness to respect, understand, and promote the mission, values, and strategic goals of CSC.
Proficient ability to collect appropriate data about human functioning, vision function, rationally analyze and evaluate this  data, and draw reasonable and valid conclusions from the data.
Proficiency in the effective and timely composition of meaningful, concise, grammatical, and accurate documentation of services conducted.
Skills in conducting diagnostic, vision, functional and psychosocial (as needed) assessments, and in using results of these assessments to develop professional service plans with measurable goals that are responsive to the needs revealed by the assessments, and to client’s preferences.
Individual Service Plan may include both billable and non-billable services to ensure a pathway to independence. Using a mix of formats including direct service, provider support, coaching, peer networks and information in both intensive programs and focused modules to provide the most robust client support. Utilize the Agency’s full range of programs to tailor a specific service plan to the client’s unique needs.
Proficiency working as a member of a service team. Proficiency in the effective, professional, respectful, and sensitive interaction with others of diverse races, ethnicities, abilities, values, strengths, beliefs, backgrounds, and personal characteristics and preferences.
Proficient use of computer applications (MS Office) for the creation and use of documents, spreadsheets, and communications.  Familiarity with electronic health and medical records. Knowledgeable in documentation of client progress in client case file. 
Comprehensive knowledge of federal and state rules, regulations, and laws pertaining to the provision of counseling and case management services, including laws and administrative rules concerning the privacy and confidentiality of protected health information. Additionally, the reporting of suspected abuse, neglect, and exploitation; and protecting the safety and health of clients and the community.
Creative and flexible problem-solving skills. Ability to promptly and appropriately change conduct and practices in response to feedback from supervision, and in response to knowledge and skills learned from training conferences at the agency, or at other organizations.

D.     Physical:  Must be able to perform frequent community travel.  Must be able to lift up to 10 pounds.
